Voor deze test wordt met een wattenstaafje een uitstrijk genomen uit de neus en keel. Bij de PCR-test een deel van het genetisch materiaal binnen in het virussterk vermenigvuldigd, om het aan te kunnen tonen. De PCR-test heeft een hoge testgevoeligheid. Dat betekent dat de PCR-test het virus ook oppikt bij iemand die nog weinig virus heeft of weinig virus meer bij zich draagt. Om de test zo gevoelig mogelijk te maken wordt eerst een specifiek stukje van het genetisch materiaal vermeerderd gekopieerd met de zogenoemde polymerase-kettingreactie, voor de meeste mensen inmiddels bekend als PCR.
De test kan dus geen positief resultaat geven bij andere virussen, zoals bijvoorbeeld een griepvirus of andere verkoudheidsvirussen. Tijdens elke cyclus wordt het genetisch materiaal verdubbeld, totdat het PCR-apparaat het detecteert. Dus hoe meer virus aanwezig is, hoe minder verdubbelingen nodig zijn om die drempelwaarde te halen. Een lagere Ct waarde betekent dan ook dat er meer virus aantoonbaar in het testmateriaal. Het verdubbelen heeft per cyclus tijd nodig.
Daarom duurt een uitslag van een PCR test altijd wat langer tenminste een paar uur. De LAMP loop-mediated isothermal amplification -test is net als de PCR-test een vorm van testen waarbij stukjes genetisch materiaal worden gekopieerd of vermenigvuldigd. Daarom is deze test sneller dan de PCR-test, die juist schommelingen in de temperatuur nodig heeft. Voor de LAMP-test zijn andere apparaten en stoffen nodig dan voor PCR-testen, zodat ze elkaar in dit opzicht bij eventuele schaarste aanvullen en niet beconcurreren.
Antigenen zijn eiwitten aan de buitenkant van het virus die een afweerreactie kunnen opwekken in het lichaam in tegenstelling tot het genetisch materiaal in het virus.
Als je het virus bij je draagt, kan een antigeentest dan al vroeg aantonen. In tegenstelling tot een PCR test, kan de antigeentest dit snel laten zien; meestal een kwartier na het opbrengen van het monster op de test-cassette.
De test is ook makkelijk buiten het laboratorium te gebruiken, en dat gebeurt dan ook veelvuldig. Omdat er geen versterking van het signaal plaatsvindt zoals bij de PCR-test, is een antigeentest minder gevoelig, wat betekent dat de test vaker een fout negatieve uitslag geeft. Vooral als er nog weinig virusdeeltjes aanwezig zijn, zoals heel vroeg na het optreden van klachten of na een bekend contact. Later in de infectie neemt het aantal virusdeeltjes weer af en dus ook de besmettelijkheid.
Ook wanneer er weinig virusdeeltjes aanwezig later de infectie kan iemand mogelijk ten onrechte een negatieve testuitslag met de antigeentest krijgen, terwijl de persoon wel COVID kan hebben.
